Your First Visit

At NDIC we strive to ensure that every patient receives an superior level of service. Our caring staff will work to make your experience as comfortable and easy as possible.

On the day of your appointment, you will need the following:

  • Photo identification
  • Insurance card
  • Prescription from your doctor’s office with diagnosis*
  • Form of payment for co-payment or co-insurance, if applicable

*Some insurance companies also require pre-authorization or a referral for your visit. Please check with your insurance provider to see if this is necessary. A list of NDIC participating insurance plans can be viewed by clicking here.

Prior to your appointment, we will need to collect some paperwork from you. We will provide these forms for you to review and complete on the day of your appointment.
